of our visitors. Our Web site contains forms through which
users may request information or supply feedback to us. In
some cases, telephone numbers or return addresses are required
so that we can supply requested information to you. Once the
information is sent to our site, it is kept in secure databases
where it is not available to users on the Internet. In some
cases, third parties assist us in maintaining these databases.

with follow-up information, but we do not provide any information

is used and disclosed. For most health care purposes,
your consent is implied as a result of your consent to treatment,
however, in some circumstances your express, sometimes written,

to the provision of your medical care.
