Scheduling an Appointment
To make an appointment with a Sleep Insights’ sleep provider, call us at 585.385.6070. If you would like someone from our office to contact you to schedule an appointment, click here. We do not require a referral from your provider to schedule an appointment.
First Appointment Documents
The following document will need to be completed at your initial appointment, unless printed and completed ahead of time:
- New Patient Packet for Rochester, Olean, and Springville
- New Patient Packet for Geneseo
- New Patient Packet for Ghaly Healing and Wellness Center
This is a copy of our HIPAA policy for your reference. It is NOT necessary to print this off and sign:
If you had a sleep study from another location or have any other pertinent records that should be available for the provider at your appointment, please complete a Release of Records Form and send it to that facility or provider so that these records can be received in our office prior to your appointment.
The Day Of Your Appointment
Please arrive 10 minutes prior to your appointment if you have not already filled out the First Appointment Documents.
Bring photo ID, insurance card(s), any prescriptions from your provider, and a method of payment for your visit. Co-pays, co-insurance or deductibles are due at the time of visit, prior to seeing the provider.
Bring your CPAP machine (sleep apnea machine) to all appointments.
Please arrive at your appointment on time. Sleep Insights requires 24 hours notice for any cancellation; otherwise, you will be charged a $50 no-show fee for office visits and $100 no-show fee for sleep studies.
At Sleep Insights, the majority of insurances are accepted, including:
- Most local commercial carriers (such as Excellus and MVP)
- Most national commercial carriers (such at Aetna and United Healthcare)
We do not accept Workers’ Compensation plans.
Sleep Insights will contact your insurance to verify coverage and obtain prior authorization, if necessary. Additionally, for sleep studies and DME (such as CPAP), we will contact your insurance to determine the portion for which you will be responsible.
If you do not have health insurance, we will provide you with our fee schedule. Please inquire with our billing staff prior to scheduling your appointment if you’d like our fee schedule.
Sleep Insights accepts MasterCard, Visa, American Express, Discover, debit cards, checks, cash, and money order. Payment can be made in person, over the phone or online.
What To Expect
If you have an upcoming appointment to see a Sleep Insights sleep specialist or have a sleep test scheduled, you can expect:
- An upscale, soothing environment
- No clinic feel, no white labs coats
- No garage or paid parking
- For overnight sleep testing, you’ll feel comfortable in our private, hotel-like bedrooms with wi-fi and cable TV
- At certain locations, there are patient lounges with breakfast/snack items and showers
