Sleep Insights will work with you to accomplish all of your testing needs. We accept all types of health insurance and will contact your patient’s insurer to determine whether our services would be covered by their particular plan. If your patient does not have health insurance or their coverage is limited with regard to our care, we will provide them with a clear and reasonable fee schedule. We are also pleased to work with other sleep centers to ensure that your patients’ sleep testing is affordable and their treatment is best suited to their needs. How to Schedule Appointments
Scheduling appointments takes two easy steps, simply call our office at (585) 385-6070, then download our referral document, fill it out and fax it to (585) 385-6071.
Referring Patients
It is important that our physicians have your patient’s medical history on the day of the consult appointment. Once your office has scheduled a consultation visit for your patient, please complete the referral document and fax it with a recent history and physical and/or clinic note to our office at (585) 385-6071.

Test Results and Next Steps
Following your patient’s study, we will schedule a meeting with them to review the results. Our clinical staff will take the time to explain the study completely and answer any questions your patient may have. Based upon the results, we will help your patient choose the best therapy moving forward. We will also mail a copy of all the results to your office, and your patient will receive a copy as well.
